Types of Vascular Lesions

Telangiectasias

Dilated superficial blood vessels visible as fine red or purple lines on the skin surface. They occur most commonly on the face (particularly the nose and cheeks) and legs. Facial telangiectasias are associated with rosacea, sun damage, and ageing. Leg telangiectasias (spider veins) are associated with venous insufficiency and hormonal factors.

Port Wine Stains

Congenital capillary malformations appearing as flat, pink to dark red patches that persist and darken with age. They are caused by a somatic mutation in the GNAQ gene and occur in approximately 0.3 percent of newborns. Without treatment, port wine stains can thicken and develop nodular components over decades.

Haemangiomas

Benign vascular tumours that are absent or minimal at birth, proliferate during infancy (typically peaking at 4-6 months), and involute spontaneously over 3-7 years. While most require only monitoring, those in critical locations (periorbital, airway) or those developing complications (ulceration, scarring) may require intervention.

Cherry Angiomas

Small, bright red papules that increase in number with age. They are extremely common and benign, occurring in up to 75 percent of people over 75. They can be easily treated when cosmetically bothersome.

Laser Treatment for Vascular Lesions

Pulsed Dye Laser (PDL)

The pulsed dye laser (585-595nm) is the gold standard for most vascular lesions. It selectively targets oxyhaemoglobin, delivering energy absorbed by the blood vessel while sparing surrounding tissue. PDL is the first-line treatment for port wine stains, facial telangiectasias, rosacea-associated redness, and superficial haemangiomas. Side effects include temporary purpura (bruising) lasting 7-14 days, though newer long-pulse PDL settings can reduce this.

Nd:YAG Laser (1064nm)

The long-pulsed Nd:YAG penetrates deeper than PDL, making it effective for deeper vessels, leg veins up to 3mm, and blue or purple lesions that are poorly targeted by 585nm wavelength. It does not cause purpura but has more discomfort during treatment and carries slightly higher risk of skin surface damage if parameters are not carefully selected.

KTP Laser (532nm)

The potassium titanyl phosphate laser is particularly effective for superficial red vessels and cherry angiomas. Its shorter wavelength targets superficial oxyhaemoglobin efficiently with minimal deep tissue effects. Treatment of cherry angiomas is typically a single session with immediate results.

Intense Pulsed Light (IPL)

IPL devices emit broad-spectrum light filtered to target vascular structures. While less specific than lasers, IPL can effectively treat diffuse facial redness, telangiectasias, and poikiloderma. It is often preferred for generalised redness where targeting individual vessels would be impractical. Multiple sessions (3-5) are typically needed.

Sclerotherapy for Leg Veins

For leg spider veins and reticular veins, sclerotherapy (injection of a sclerosing agent) remains the gold standard. It is more effective than laser for leg veins larger than 1mm and can be combined with laser for comprehensive treatment of the legs. Foam sclerotherapy extends the application to larger reticular veins up to 6mm.

How many laser sessions are needed for port wine stains?

Port wine stains typically require 8-15 or more PDL sessions spaced 6-8 weeks apart for significant improvement. Lighter lesions respond better than darker ones. Complete clearance is rarely achievable, but significant lightening (50-80 percent improvement) is typical. Treatment is most effective when started in early childhood.

Can rosacea redness be permanently treated?

Laser and IPL can significantly reduce the background redness and visible vessels of rosacea. However, rosacea is a chronic condition, and the underlying tendency to develop new vessels persists. Maintenance treatments every 6-12 months help manage recurrence. Combining laser with medical rosacea management produces the best long-term results.

Is laser treatment for veins painful?

PDL feels like a rubber band snap with brief stinging. Nd:YAG laser is more uncomfortable due to deeper penetration. Cooling devices and topical anaesthetic manage discomfort. Most patients tolerate treatment well, and sessions are relatively brief at 15-45 minutes depending on the area.

Do treated blood vessels grow back?

Vessels that are completely destroyed do not regrow. However, the underlying condition causing vessel formation typically persists, meaning new vessels can develop over time. Maintenance treatments address new vessel formation while preserving previous treatment results.
