Fractional Laser Resurfacing

Overview

Fractional laser resurfacing treats a fraction of the skin surface in a grid of microscopic treatment zones, leaving surrounding tissue intact to accelerate healing. It can be delivered by ablative lasers (which remove tissue, such as CO2 and erbium) or non-ablative lasers (which heat the dermis without breaking the surface).

Ablative versus non-ablative

Ablative fractional treatment gives a stronger result per session with visible healing over roughly five to seven days. Non-ablative fractional treatment has far less downtime, usually redness for a day or two, but requires a course of sessions and produces a more gradual change. Neither is universally better; the choice depends on the concern, skin type and how much recovery is acceptable.

What is involved

  • Course: one to three sessions for ablative; commonly three to six for non-ablative, spaced about a month apart.
  • Anaesthetic: topical anaesthetic, sometimes with cooling.

Risks and limitations

Redness, swelling and a sandpaper texture during healing are expected. Recognised risks include post-inflammatory hyperpigmentation, infection, prolonged redness and, less commonly, scarring. Risk of pigmentary change rises with darker skin types and with sun exposure before or after treatment.

Results build over three to six months as collagen remodels, so judging the outcome immediately after a course is premature.
