Facelift (Rhytidectomy)

Overview

A facelift lifts and repositions the deeper tissues of the face and neck and removes excess skin, addressing sagging along the jawline and in the mid-face. Modern techniques generally reposition the underlying SMAS layer rather than tightening skin alone, which is what produces a pulled appearance when done poorly.

What is involved

  • Anaesthetic: general anaesthetic, or local with sedation for more limited procedures.
  • Duration: commonly 2 to 5 hours depending on extent.
  • Stay: often one night.

Recovery

Bruising and swelling are substantial in the first two weeks. Most people take two to four weeks away from work and social commitments. Numbness around the incisions and ears is normal and can persist for months. Final results are usually judged at six months or later.

Risks and limitations

Recognised complications include haematoma (a collection of blood requiring drainage, and the most common early complication), injury to branches of the facial nerve causing weakness that is usually temporary but occasionally permanent, numbness, hair loss along the incision, and visible or thickened scarring.

A facelift does not improve skin texture, pigmentation or fine surface lines, and it does not stop ageing. It repositions tissue; it does not restore lost volume, which is why it is sometimes combined with other procedures.

Who it may not suit

Smoking is the single biggest modifiable risk factor for skin necrosis after a facelift, and many surgeons will not operate on current smokers. Uncontrolled hypertension increases haematoma risk. Realistic expectations about longevity matter: the face continues to age after surgery.
