De Quervain’s Release
Overview
De Quervain’s tenosynovitis is inflammation of the sheath surrounding two tendons on the thumb side of the wrist. It causes pain over the wrist that is worse on gripping, lifting or turning the wrist, and it is common in new parents and after repetitive thumb use.
Before surgery is considered
First-line management is a thumb spica splint, activity modification and anti-inflammatory medication. A corticosteroid injection into the tendon sheath is often effective and many people need nothing further.
What the operation involves
Surgical release opens the first extensor compartment through a small incision at the side of the wrist, freeing the tendons.
- Anaesthetic: local anaesthetic.
- Duration: typically 15 to 20 minutes.
- Stay: day case.
Recovery
Movement is encouraged early. Sutures are removed at around 10 to 14 days. Most people regain comfortable function over four to six weeks, though scar sensitivity can persist longer.
Risks and limitations
The main specific risk is injury to branches of the superficial radial nerve, which cross the operative field and can leave numbness or a painful neuroma on the back of the thumb. Other risks include infection, scar tenderness, incomplete relief, and tendon subluxation if the compartment is released too widely.

Recovery Timeline
D1
Day 1: Immediately After
Mild redness and swelling at the treatment area is normal. Apply cool compresses as advised by your practitioner. Avoid touching the treated area.
D3
Days 2-3: Initial Healing
Swelling begins to subside. You may experience mild bruising which can be concealed with makeup. Resume normal daily activities with care.
W1
Week 1: Visible Improvement
Initial results become visible. Any bruising should have resolved. Follow-up check with your practitioner to assess progress and address any concerns.
W4
Week 4: Full Results
Treatment has fully settled with optimal results now visible. Final review appointment to evaluate outcome and discuss maintenance programme.
