CO2 Laser Resurfacing

Overview

CO2 laser resurfacing uses a carbon dioxide laser, at a wavelength of 10,600 nm, absorbed by water in the skin. It removes (ablates) tissue in a controlled way and heats the layer beneath, prompting collagen remodelling over subsequent months. It is among the most effective resurfacing options available and correspondingly among the highest risk.

Ablative and fractional

Fully ablative treatment removes the entire surface of the treated area, producing the strongest result and the longest recovery. Fractional CO2 treats a grid of microscopic columns, leaving untreated skin between them to speed healing. Most current practice is fractional, and depth and density are adjustable, which is why the same device can deliver very different treatments.

What it is used for

Acne scarring, surgical and traumatic scars, deep static lines particularly around the mouth and eyes, sun damage and uneven texture.

Recovery

Expect redness, swelling, oozing and crusting. Fractional treatment typically involves five to seven days of visible healing, then residual pinkness that can persist for weeks to a few months. Fully ablative treatment involves substantially longer recovery. Strict sun avoidance afterwards is essential.

Risks and limitations

Recognised complications include prolonged redness, post-inflammatory hyperpigmentation, permanent hypopigmentation, infection including reactivation of cold sores (antiviral cover is usual), scarring, and ectropion if treatment around the lower eyelid is too aggressive.

Skin type is the dominant safety factor. Higher Fitzpatrick types carry a materially greater risk of pigmentary change, and settings must be adjusted accordingly or a different modality chosen. A test patch is reasonable to request.

Who it may not suit

Generally avoided in pregnancy, in active skin infection, with a recent course of oral isotretinoin, in people with a history of keloid scarring, and where sun exposure cannot be avoided afterwards.

Skin type assessment and discussion of treatment depth. Pre-treatment skincare may be prescribed for 2-4 weeks prior.

Anaesthesia

Topical anaesthetic cream applied 45-60 minutes before treatment. Nerve blocks may be used for deeper treatments.

The fractional CO2 laser is passed over the treatment area in a precise pattern. Cooling is used throughout for comfort.

Recovery Timeline

D1

Day 1: Immediately After

Mild redness and swelling at the treatment area is normal. Apply cool compresses as advised by your practitioner. Avoid touching the treated area.

D3

Days 2-3: Initial Healing

Swelling begins to subside. You may experience mild bruising which can be concealed with makeup. Resume normal daily activities with care.

W1

Week 1: Visible Improvement

Initial results become visible. Any bruising should have resolved. Follow-up check with your practitioner to assess progress and address any concerns.

W4

Week 4: Full Results

Treatment has fully settled with optimal results now visible. Final review appointment to evaluate outcome and discuss maintenance programme.
