Fat dissolving injections have become one of the most talked-about treatments in aesthetic medicine, promising to eliminate stubborn fat deposits without surgery. At Axiom Aesthetics, we offer evidence-based fat dissolving treatments while providing honest guidance about what these products can and cannot achieve — including some important warnings about certain popular products.

How Fat Dissolving Injections Work

The Science of Injectable Lipolysis

Fat dissolving injections contain substances that destroy fat cells (adipocytes) through a process called adipocytolysis. When injected into a localised fat deposit, the active ingredient disrupts the membrane of the fat cell, causing it to rupture. The released fatty contents and cellular debris are then processed by the body’s lymphatic and immune systems and eliminated naturally over the following weeks.

The key principle is that destroyed fat cells do not regenerate. Unlike weight loss through diet (which shrinks fat cells but does not eliminate them), injectable lipolysis permanently removes the treated fat cells from the body. However, remaining fat cells in the area can still enlarge with weight gain, so maintaining a stable weight is important for preserving results.

The Inflammatory Response

Fat dissolving injections work by inducing a controlled inflammatory response in the target tissue. This inflammation is both the mechanism of action and the cause of the significant swelling that characterises the post-treatment period. Understanding and accepting this swelling is essential for patient satisfaction.

Available Products

Deoxycholic Acid-Based Products

Aqualyx

Aqualyx is a CE-marked, medical-grade fat dissolving product containing a modified form of deoxycholic acid (12-alpha-hydroxy-5-beta-24-oic sodium salt). It has been available in Europe since 2009 and has the most extensive clinical data of any fat dissolving product available in the UK.

  • Active ingredient: 7.5% deoxycholic acid derivative
  • Regulatory status: CE-marked medical device (Class III)
  • Clinical evidence: Multiple peer-reviewed studies, including randomised controlled trials
  • Treatment areas: Submental fat (double chin), jowls, small body fat deposits (flanks, inner thighs, bra fat, banana roll)
  • Sessions required: Typically 2-4 sessions spaced 4-6 weeks apart
  • Results timeline: Visible reduction from 4-6 weeks after each session; final results 2-3 months after last session

Kybella/Belkyra

Kybella (known as Belkyra in the UK) is an FDA-approved formulation of synthetic deoxycholic acid, specifically licensed for the treatment of submental fat (double chin). It is identical in mechanism to Aqualyx but has the distinction of being the only fat dissolving product with FDA approval for a cosmetic indication.

  • Active ingredient: 1% deoxycholic acid
  • Regulatory status: FDA-approved (USA); available in UK
  • Treatment area: Submental fat only (FDA-approved indication)
  • Sessions required: Typically 2-6 sessions spaced at least 1 month apart

Lemon Bottle: An Important Warning

Lemon Bottle has become enormously popular on social media, marketed as a “painless” and “swelling-free” fat dissolving treatment. However, there are significant concerns that patients should be aware of:

  • Not a regulated medical product: Lemon Bottle is classified as a cosmetic product, not a medical device or pharmaceutical. This means it has not undergone the rigorous testing required of medical products
  • Questionable mechanism: The manufacturer claims the product dissolves fat through a blend of riboflavin (vitamin B2), bromelain (pineapple enzyme), and lecithin. While these are recognised substances, the evidence that they can effectively dissolve fat when injected at the concentrations used is limited
  • Lack of clinical trials: Unlike Aqualyx and Kybella, there are no published peer-reviewed clinical trials demonstrating Lemon Bottle’s efficacy or safety profile
  • Safety concerns: The British College of Aesthetic Medicine (BCAM) and Save Face have both raised concerns about the product’s regulatory status and the lack of evidence supporting its use
  • The “no swelling” claim: Paradoxically, the lack of swelling may indicate that the product is not causing sufficient adipocytolysis (fat cell destruction) — because effective fat dissolving inherently causes an inflammatory response

At Axiom Aesthetics, we do not offer Lemon Bottle treatments. We believe in using only products with robust clinical evidence and appropriate regulatory status. While we understand the appeal of its marketing claims, patient safety must always take precedence.

Other Products

Several other fat dissolving products exist in various markets:

  • Lipodissolve/Lipostabil: An older phosphatidylcholine/deoxycholate combination that preceded modern products. Still used in some markets but largely superseded
  • Various “fat dissolving” cocktails: Some practitioners use compounded mesotherapy cocktails for fat reduction. These are unregulated and lack standardised evidence

Treatment Areas and Suitability

Best Treatment Areas

Fat dissolving injections are most effective for small, localised fat deposits that are resistant to diet and exercise:

  • Submental fat (double chin): The most studied and most popular treatment area, with the strongest evidence base
  • Jowl fat: Small fat deposits contributing to jowling
  • Bra fat: Fat deposits around the bra line
  • Flanks/love handles: Small, pinchable fat deposits on the sides of the torso
  • Inner thighs: Localised fat deposits on the upper inner thighs
  • Banana roll: The crescent-shaped fat deposit beneath the buttocks
  • Upper arms: Small deposits resistant to exercise

Areas to Avoid

  • Large, diffuse fat deposits (not suitable — consider liposuction)
  • Areas near important nerves (marginal mandibular nerve in the jawline area)
  • Visceral fat (the product treats subcutaneous fat only)
  • Areas with poor lymphatic drainage

Who Is a Good Candidate?

  • Patients at or near their ideal body weight with localised, stubborn fat deposits
  • BMI ideally under 30 (fat dissolving is not a weight loss treatment)
  • Those who can tolerate significant swelling for 1-2 weeks per session
  • Patients with good skin elasticity in the treatment area (loose skin may not retract after fat reduction)
  • Non-smokers with realistic expectations

Contraindications

  • Pregnancy and breastfeeding
  • Active infection or inflammation in the treatment area
  • Autoimmune conditions
  • Severe liver or kidney disease
  • Lipodystrophy or pathological fat conditions
  • Blood clotting disorders or anticoagulant therapy

What to Expect: The Treatment Process

Before Treatment

  • Comprehensive consultation with assessment of the fat deposit and skin quality
  • Discussion of realistic outcomes and treatment alternatives
  • Clinical photographs for before-and-after comparison
  • Consent and medical history review

During Treatment

  1. The treatment area is marked and measured
  2. Local anaesthetic is applied (topical or injectable)
  3. The fat dissolving solution is injected through a series of small injections across the treatment area (typically using a fine needle)
  4. Multiple injection points are used to distribute the product evenly throughout the fat deposit
  5. Treatment takes approximately 15-30 minutes depending on the area

After Treatment

The post-treatment period is the most challenging aspect of fat dissolving injections:

  • Immediate: Redness, warmth, and stinging at the injection site (normal)
  • Days 1-3: Significant swelling — often described as “worse before better.” The treated area can swell dramatically (particularly the submental area, which can resemble a “bullfrog” appearance). This is entirely normal and expected
  • Days 3-7: Swelling begins to reduce. Bruising may develop. Some patients experience numbness or tingling
  • Weeks 2-4: Swelling continues to subside. Early fat reduction may become visible
  • Weeks 4-8: True results become apparent as the body processes the destroyed fat cells
  • 8+ weeks: Full results visible; assessment for additional sessions

Results and Expectations

What Results Can I Expect?

Clinical studies on Aqualyx show:

  • Average circumference reduction of 1.5-3cm per treatment session in the submental area
  • Patient satisfaction rates of 70-85% after completing a treatment course
  • Permanent fat cell destruction — once destroyed, the cells do not return
  • Optimal results typically require 2-4 sessions

Managing Expectations

Fat dissolving injections are best understood as a refinement tool rather than a dramatic body contouring solution. They are ideal for addressing small, localised deposits that bother patients despite good overall fitness. Patients expecting dramatic, surgical-level results are likely to be disappointed. During your consultation, we provide honest guidance about what is achievable.

Frequently Asked Questions

How painful are fat dissolving injections?

The injection process itself causes moderate discomfort — a stinging or burning sensation as the product is introduced. Local anaesthetic (topical cream and/or injectable lidocaine) significantly reduces this. After injection, patients typically experience a warm, throbbing sensation for 1-2 hours. Paracetamol is usually sufficient for any post-treatment discomfort. Most patients rate the discomfort as tolerable, particularly knowing it is temporary.

Is the swelling really as bad as people say?

The swelling from effective fat dissolving treatments is significant and should not be underestimated. In the submental area (double chin), the swelling can make the area appear temporarily larger than before treatment — this can be quite alarming if you are not prepared. We always show patients photographs of typical post-treatment swelling during the consultation so they know exactly what to expect. Planning treatment timing carefully (avoiding important events for 2-3 weeks post-treatment) is essential.

Why do you not offer Lemon Bottle?

At Axiom Aesthetics, patient safety is our highest priority. While Lemon Bottle is widely marketed and popular, it lacks the clinical evidence, regulatory status, and safety data that we require before offering a treatment. We believe patients deserve treatments backed by peer-reviewed research and appropriate regulatory oversight. We will consider adding Lemon Bottle or similar products if and when sufficient clinical evidence becomes available.

Can the fat come back after treatment?

The specific fat cells destroyed by the treatment are permanently eliminated. However, remaining fat cells in the treated area can enlarge with weight gain, and significant weight gain can create new fat deposits elsewhere. Maintaining a stable, healthy weight is important for preserving results. If you gain a small amount of weight (2-3kg), the treated area will typically still appear improved compared to the pre-treatment state.

How many sessions will I need?

Most patients require 2-4 sessions for optimal results, though this varies based on the size and density of the fat deposit and individual response. We assess results at 6-8 weeks after each session before planning the next. Some patients achieve satisfactory results after just 2 sessions, while others may benefit from 4. During your consultation, we provide an estimated treatment plan, though the final number of sessions is always guided by your individual response.
