What Are Biostimulatory Fillers?

Biostimulatory fillers represent a distinct category of injectable treatments that go beyond simple volumisation. Unlike hyaluronic acid fillers that provide immediate volume through a gel matrix, biostimulators work by triggering the body’s own collagen production, gradually building tissue volume and improving skin quality over weeks to months. The result is a more natural, progressive improvement that patients and practitioners increasingly favour for facial rejuvenation.

Poly-L-Lactic Acid (Sculptra)

Mechanism of Action

Sculptra consists of microparticles of PLLA suspended in a carrier solution. When injected subdermally, the carrier is absorbed over days, while the PLLA particles stimulate a controlled foreign body response. Macrophages and fibroblasts encapsulate each particle, producing new type I collagen over a period of weeks. The PLLA gradually biodegrades over 18-24 months, but the collagen it stimulated persists.

Clinical Applications

Sculptra excels at treating global facial volume loss, particularly in the temples, cheeks, and pre-auricular areas. It is the treatment of choice for severe lipoatrophy (as seen in HIV-associated facial wasting) and is increasingly used for non-surgical facial rejuvenation seeking natural, gradual improvement.

Treatment Protocol

Typically 2-4 sessions spaced 4-6 weeks apart. Results develop over 3-6 months as collagen builds. The reconstitution volume and injection technique are critical for avoiding nodule formation. Post-treatment massage (the 5-5-5 rule: massage 5 times daily for 5 minutes for 5 days) helps ensure even distribution.

Calcium Hydroxylapatite (Radiesse)

Mechanism of Action

Radiesse contains calcium hydroxylapatite (CaHA) microspheres (30 percent) in a carboxymethylcellulose gel carrier (70 percent). It provides dual benefit: immediate volume from the gel carrier plus long-term biostimulation as the microspheres stimulate neocollagenesis. The CaHA is gradually metabolised into calcium and phosphate ions, leaving behind a collagen matrix.

Hyperdilute Radiesse

A revolutionary application involves diluting Radiesse with saline and/or lidocaine to create a biostimulatory preparation for broad skin quality improvement rather than focal volumisation. Hyperdilute CaHA is injected superficially across large areas (face, neck, hands, decolletage) to stimulate diffuse collagen production, improving skin thickness, elasticity, and quality.

Treatment Areas

Standard Radiesse: cheeks, jawline, chin, hands (volumisation). Hyperdilute Radiesse: face, neck, decolletage, arms, knees, abdomen (skin quality improvement).

Emerging Biostimulators

Polycaprolactone (Ellanse)

PCL microspheres in a carboxymethylcellulose carrier offering immediate volumisation plus sustained biostimulation. Available in different longevity profiles (S: 1 year, M: 2 years, L: 3 years, E: 4 years) depending on the ratio of PCL to carrier. The longer-lasting formulations are particularly popular for structural support in the midface and jawline.

Polynucleotide Biostimulators

While primarily skin boosters, polynucleotide preparations also function as biostimulators through their activation of purinergic receptors and promotion of fibroblast proliferation. They represent the gentlest end of the biostimulation spectrum, suitable for delicate areas where more aggressive biostimulators carry higher risk.

Choosing the Right Biostimulator

  • Sculptra: Best for global volume restoration and patients wanting very gradual, natural improvement
  • Radiesse (standard): Best for immediate structural support with ongoing stimulation (jawline, cheeks, hands)
  • Radiesse (hyperdilute): Best for broad skin quality improvement (neck, decolletage, body)
  • Ellanse: Best for patients wanting longer-lasting results with immediate and progressive improvement

Safety Considerations

Unlike HA fillers, biostimulatory fillers cannot be dissolved with hyaluronidase. This irreversibility demands precise injection technique and conservative treatment planning. Nodule formation is the most common complication, minimised by proper reconstitution, appropriate injection depth, and post-procedure massage. Biostimulators should not be used in the lips or periorbital area due to higher nodule risk.

Frequently Asked Questions

How long do biostimulatory results last?

Sculptra: up to 2 years. Radiesse: 12-18 months. Ellanse: 1-4 years depending on formulation. Results are maintenance-based, with top-up sessions extending longevity.

Can biostimulators be combined with HA fillers?

Yes, they are highly complementary. Biostimulators restore foundational volume and skin quality, while HA fillers provide precise contouring and correction of specific lines or hollows.

Are biostimulatory fillers natural-looking?

The gradual onset of results (developing over weeks to months) produces very natural-looking improvement, avoiding the sudden change associated with large-volume HA filler treatment.
